This waiver must be submit by the sponsoring current member for the guest and accompanied by a $25 guest fee to be paid upon entering the studio. Cash or checks are accepted.

The $25 fee covers creation of up to a maximum of 3 finished pieces.
Guests may ONLY use the studio when accompanied and supervised by a Potters' Club member.

Waiver and release of all claims and permission to secure treatment

Please read carefully and be aware that in utilizing the Potters' Club studio you will be waiving and releasing all claims for injuries you and your dependents might sustain arising from use of Potters' Club equipment and facilities, and accepting responsibility for all claims against the Club for injuries your guests might sustain.

I recognize and acknowledge that there are certain risks of physical injury to participants in the Potters' Club, and I agree to assume the full risk of any injuries, damages, or loss regardless of severity which might be sustained by myself, my dependents, or any guests I might bring to the Potters' Club studio or events as a result of participating in any and al l activities connected with or associated with the Potters' Club, or with the use of any Potters' Club facility or equipment.
I agree to waive and relinquish all claims against the Potters' Club and its officers, agents, servants, and employees which I, my dependents, and guests may have as a result of participating in the program. I do hereby fully release and discharge the Potters' Club and its officers, agents, servants, and employees from any and all claims from injuries, damage, or loss which I, my dependents, or guests may have or which may accrue to me or my dependents or guests and arising out of, or connected with, or in any way associated with the activities of the Potters' Club or with the use of any Potters' Club facility or equipment.
I further agree to indemnify and hold harmless and defend the Potters' Club and its officers, agents, servants, and employees from any and all claims resulting from injuries, damages, and losses sustained by me, my dependents, and guests arising out of, connected with, or in any way associated with the activities of the Potters' Club or with the use of any Potter's Club facility or equipment.
I acknowledge that my guest privileges do not extend to my dependents or any guests I might bring into the studio. I acknowledge that I take full responsibility for any guests I bring into the Potters' Club studio and take responsibility from the Club from any claim resulting from injuries, damages, and losses sustained by my guests arising out of, connected with, or in any way associated with the activities of the Potters' Club or with the use of any Potters' Club facility or equipment I also acknowledge and accept that I wi ll be held responsible by the Club for damage to Club property resulting from flagrant or intentional misuse of Club facilities or equipment by myself, my dependents, or my guests.
In the event of an emergency, I authorize the Potters' Club to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for me or my dependents' or guests' immediate care and agree that I will be responsible for payment of all medical services rendered.
